The previous response has the red document icon means that the responses coming from Assessment form but not automatically update the care plan. Which mean the responses will be there after you create or re-create a care plan.

To update the setting for Assessment form automatically update on Care Plan:

  1. Click Admin, then click Create/Edit form.

  2. Find the assessment form then click on its name.

  3. Change on Update Care Plan to update active care plan.
