Dynamic care plans

Dynamic care plan is a feature that you can enable per assessment form. When you enable this, each time you complete an assessment form, the responses are captured and automatically added to the resident’s active care plan, ensuring the care plan is always showing the latest information.

Each time new information enters the care plan, the previous information is automatically strikes through, however maintains this for audit purposes. You can view the struck through information when you evaluate the care plan.


Set the assessment form to update

The first thing you need is to set your assessment form to update the care plan.

  1. Click Admin.

  2. Under Clinical configuration, click Create/edit form.

  3. Click the name of the form you want to change.

  4. On the Update careplan list, select one of these two options below.

  5. Click Save.

When you complete the assessment form, any responses mapped to a care domain or sub-domain automatically updates the care plan.

When you complete the assessment form, it asks you whether you’d like the information to automatically update the care plan. If you say yes, the responses mapped to a care domain or sub-domain automatically updates the care plan.


Map the questions to the care plan

Once you’ve set your assessment form to update the care plans, the next thing to do is map the questions to the care domains or sub-domains.

  1. Click Admin.

  2. Under Clinical configuration, click Create/edit form.

  3. To the right of the form you want to change, click Edit.

  4. Expand the question you wish to map by clicking the down arrow.

  5. If required, expand the mapping section by clicking the down arrow.

  6. Under the appropriate section, use the Care domain and Sub domain lists to select the relevant area to map to.

  7. Repeat this for any other questions you need on the assessment form. To finalise, click Save.


Dynamic care plan behaviour

Where you have dynamic care planning in place, you may need to be aware of actions taking place and their effect on the care plan. Below is a small list of the common areas to be aware of.

Change a care domain

If you map the assessment form response to a different care domain or sub-domain from the one initially set, the data in the active care plan immediately changes and displays in the new domain.

Minor corrections

Any corrections to spelling or grammar automatically reflects in the care plan if there’s been no change to the assessment form version.

Archive a complete assessment form

If you archive a complete assessment form, the mapped data remains on the active care plan.

Archive an assessment form template

If you archive an assessment form template, the mapped data strikes through with your username and action date.

Map to a non-active care domain

If a mapped care domain isn’t currently active on the care plan you’re completing the assessment for, the care domain automatically displays, together with the assessment form data.
