Evaluating your care plans

Evaluating your care plans ensures that the care is current and correct for each resident, ensuring you’re delivering the best care to your residents. You can evaluate the whole care plan, the care domains that make up the care plan, and the observations, goals and interventions within them.


Navigate to the care plan

The first step in evaluating your care plan is to get to the evaluation screen.

  1. Click Residents, then click on the resident you’re reviewing.

  2. Click the Careplan tab, then click Evaluate.


Evaluate care domains

The care domains show the specific areas of the care plan. You can expand each care domain by clicking the down arrow.

  1. Expand the care domain by clicking the down arrow.

  2. Review the content in the Observations, Goals and Intervention sections.

  3. On the care domain heading, click Evaluate care domain.

  4. On the Outcome list, select the appropriate outcome.

  5. Add your evaluation notes, then click Save.

When saving, a new progress note adds to the care domain Evaluation notes section and the last evaluation date displays on the care domain header.


Evaluate goals and interventions

You can view and evaluate the goals and interventions for each care domain individually, also giving you opportunity to add, move link or strike through entries.

  1. Expand the care domain by clicking the down arrow.

  2. On the goal or intervention note, click Evaluate.

  3. On the Outcome list, select the appropriate outcome.

  4. Add your evaluation notes, then click Save.

When saving, a new note adds to the goal or intervention with your evaluation notes.



Add an entry manually

If your care domain is missing entries, you can add them now.

  1. Click the Add New button to the right of whichever section you’re adding to.

  2. Fill in the information required, then click Save.


Move an entry

If your entries are in the wrong order, you can correct that now.

  • Drag the grey three grey lines on the entry.


Link a goal to an intervention

If a there should be a link between a goal and intervention, you can update it now.

  • Drag the intervention item to the goal item using the three grey lines.

Once complete, a link icon appears.


Strike through an observation, goal, intervention or evaluation note

If there’s information to remove, you can use the strike through feature. This leaves the original content in place, but displays it as struck through.

  1. Click Strike through on the note.

  2. Enter a reason you’re striking it through, then click Save.

  3. To view the struck notes, select Show care domain strike throughs.


View assessment form

Where your notes have associated forms, you can view them while evaluating the care plan.

  1. Point to the blue or red note icon on the observation, goal or intervention.

  2. On the pop-up details, click the assessment link.


Finalise your evaluation

After completing the evaluation on the care domains and any corresponding notes, you can finalise the evaluation of the whole care plan.

  1. Under Careplan, click Evaluate careplan.

  2. On the Outcome list, select the appropriate outcome.

  3. Enter your evaluation notes, then add a review date.
    Note: The review date must be equal to, or greater than today’s date.

  4. Click Save.

Once you save, a progress note adds under Evaluation notes and the care plan header shows the last evaluation date.
