You can edit your assessment forms to ensure that they’re always current and capturing the right information, giving you the right tools to provide the best care.


Navigate to your assessment form

Before you can review and edit your form, you need to get to the edit screen.

  1. Click Admin.

  2. Under Clinical Configuration, click Create/edit form.

  3. Find the assessment form you need to change, then click Edit.


Add a new question

Adding a new question to your form is easy, with a single form guiding you through your question requirements. Most options are not mandatory, so you don’t need to select or enter into every box.

  1. To add a new question to your assessment form, click Add a question.

  2. Enter your question and select the options you require.

Mandatory question

Select to mark the question as mandatory, ensuring that it needs an answer on every assessment.

Bold text

Select to display the question text in bold.

Red text

Select to display the question text in red.

Message

Enter a custom note help explain what information the answer needs.

  1. On the Response type list, select the type of data you need for the answer.

  2. Map your question to a care domain using the Care domain and Sub domain lists.


Drop-down and Multi-select responses

For drop-down and multi-select responses, you can populate the lists with your own choices, and give those choices a value that totals at the end of the form.

  1. Under Dropdown choices, click Add a choice.

  2. Enter the name of the of the choice.

  3. Enter a score for that choice.

You can repeat this for every choice you need. To reorder them, drag on the three grey lines.


Remove a question

If you no longer need a question, you can remove it from the assessment form.

  1. Expand the question by clicking the down arrow.

  2. Click Delete question then click Delete question.


Edit an existing question

If you just need to adjust an existing question, you can do so easily.

  1. Expand the question by clicking the down arrow.

  2. Next to the question, click Edit.

  3. Enter the new question text.

  4. Answer whether the meaning of the question has changed.

  5. Click Done.


Finalise your changes

The final step before your changes are complete is to save your assessment form. Before doing so, it’s always worth doing a preview to ensure that the changes are what you’re looking for.

  1. Click Preview Form and ensure your changes are correct.
    Tip: You can change the order of the questions by dragging the three grey lines.

  2. When you’re happy, click Save.


Previous Versions

Once a form template has been edited, it is not possible to roll-back to the previous version, instead the new version must be re-edited manually to undo any changes that were made.
