Accurate fluid balance reporting is essential for monitoring residents’ hydration and overall health. The Care & Clinical system calculates fluid totals based on the date an observation is entered, which means timing of data entry can significantly impact report accuracy.

  • Fluid intake totals are calculated per calendar day.

  • If a fluid observation is entered on the following day, it will be counted toward that next day’s total, not the original day.


Why Reports May Show Incorrect Totals

During audits comparing the Care & Clinical system report with manual checks, discrepancies were found. These often show residents as under target or occasionally over target.

Possible reasons for inaccuracies:

  • Retrospective entries: If staff enter fluid observations after the 24-hour period, the system assigns them to the date they were entered, not the date they occurred.

  • This can lead to totals being misaligned with actual intake, especially if data entry is delayed.


Best Practices for Accurate Fluid Recording

  1. Enter observations promptly – Record fluid intake on the same day it occurs.

  2. Avoid retrospective entries – If unavoidable, note the actual time and date in comments.

  3. Verify totals regularly – Use the report to check for anomalies and correct errors quickly.

  4. Train staff on system logic – Ensure everyone understands how the system calculates daily totals.
